Job Description
Geriatric Long Term care with short term rehab facility. Beautiful plush rural area of Connecticut.

- Develop individualized care plans based on resident assessments, including medical needs, dietary requirements, personal care needs, and psychosocial considerations.
- Monitor and regularly review care plans to ensure effectiveness and make necessary adjustments.
- Staff Supervision and Training:
- Supervise Certified Nursing Assistants (CNAs), Licensed Practical Nurses (LPNs), and other nursing staff.
- Assign tasks and delegate responsibilities to nursing staff based on resident needs and staff competency.
- Provide ongoing training and education to nursing staff on new procedures, policies, and best practices.
- Address performance issues and provide corrective action when necessary.
- Quality Assurance and Compliance:
- Monitor resident care delivery to ensure compliance with all state and federal regulations.
- Conduct quality assurance reviews and identify areas for improvement
- Implement corrective actions to address any identified quality concerns
- Communication and Collaboration:
- Communicate effectively with residents, families, physicians, other healthcare professionals, and facility staff
- Collaborate with other departments within the facility to coordinate resident care
- Address concerns and complaints from residents, families, and staff in a timely and professional manner
- Administrative Tasks:
- Assist the Director of Nursing with administrative duties such as scheduling, staff assignments, budget management, and report generation
- May assume responsibilities of the Director of Nursing in their absence
Required Qualifications: - Registered Nurse (RN) license in good standing
- Experience in long-term care setting preferred
- Strong leadership and supervisory skills
- Excellent communication and interpersonal skills
- Ability to manage multiple priorities and work effectively under pressure
- Knowledge of state and federal regulations regarding long-term care facilities is preferred
- 3 years Prior in RN Supervisor capacity
- Associates Degree acceptable, BSN preferred
- Good Electronic Medical Records documentation.

- $102-115K-depending on experience
