In the Role of the Case Coordinator/Case Manager you will help reduce readmission risks by focusing on improving care coordination and eliminating gaps in care that lead to unnecessary readmissions.

Improving outcomes. Improving patient outcomes, by scheduling follow up appointments with PCP and interacts with providers about care delivery and play an integral role in guiding physicians in their decisions about the most appropriate level of care for patients.

Evaluate whether the level of documentation regarding the severity of illness justifies the intensity of service provided while ensuring that each patient gets appropriate and timely care.

  The Case Coordinator/ Case managers ensures that any patient' s hospital stay, or any part of that stay, is medically necessary, delivered in the most appropriate setting and is not custodial in nature.

The Case Coordinator notifies and educates the patient about insurers' decisions regarding medical necessity and ensure that the patient receives a Hospital-Issued Notice of Non-coverage and  advocates on the patient' s behalf to get the insurer to reverse its decision, if possible

These preventive care programs are designed for at-risk populations who have costly chronic conditions such as asthma, chronic obstructive pulmonary disease, diabetes, depression and heart failure.

With Excellent communication skills, make a significant difference by motivating patients to improve their health status and potentially reduce hospitalizations

Case manager in care coordination, transitions of care, and evaluation of quality measures will have to evolve to meet the facility' s needs.

Discharge planning and transitions of care alongside on decisions to help prevent readmission are recent additions to the duties of the case which involves playing multiple roles, including but not limited to utilization review, coordination of care between facilities and providers, benefits determinations, payer interfaces, and care management.

Coordinates home care or other services and uses external resources

Reviews insurance authorization and evaluates appropriateness for admission.
